Evaluations
If there are evaluation forms you can create, a page for Evaluation Forms will be available for you to view and create evaluation forms. These forms are created by your school program. They may be self-evaluations, or evaluations you send to preceptors, instructors, or school accounts if required. Forms you send may have areas you need to complete prior to sending as well.
Please note: Some installations allow other users to create evaluation forms to evaluate you without you sending them. It is important to check your Notifications page or other Evaluation pages to make sure you confirm viewing evaluations that require confirmation.
Some installations may have different menu options where students can find evaluation forms they can initiate.

Sent Evaluations
The My Evaluations page appears when you have sent evaluations. You will find them here along with the status of the evaluation. Some forms will require you to ‘confirm’ viewing the form, pay attention to the labels given for each evaluation.
For some installations you sent evaluations are found on the same page where you created the evaluation form.
Examples: Daily Evaluations, Other Evaluations
The table can also be filtered for many things, including: evaluation status, draft status, confirmation status, and by evaluation form title.
Formal Evaluations
If any of your evaluation forms can have a formal report generated for it by your faculty or instructor, it will appear on the My Formal Evaluations page. These forms are not initiated by students.
Formal evaluations typically require a comment to be added to confirm viewing the evaluation.
